Old Colony Planning Council — Area Agency on Aging

Designated Area Agency on Aging for 23 communities in southeastern Massachusetts. Coordinates services including in-home assessments, care plans, home care, and caregiver support for the South Shore and surrounding areas.

Aging Services Access Points (ASAPs)
